- Note:
A RGA pre-authorization number must be obtained before parts are returned
for tracking and processing purposes.
- Please print two
(2) copies of this form. Keep one for your records and use one
as a packing slip for defective parts being returned to V & H,
Inc.
- Please use the Warranty Claims Consideration form as a packing slip with
all other related parts and labor invoices to process the claim.
- Return all information within 30 days after repair is completed.
- Remember, all defective
parts must be shipped back, freight prepaid, to:
